Consent to the processing of personal data

Hereby, in accordance with Federal Law No. 152-FZ dated July 27, 2006 “On Personal Data,” freely, by my own will, and in my own interest, I provide my consent (hereinafter – the “Consent”) to the processing of my personal data to the Operator – Ural Wood Limited Liability Company (Ural Wood LLC), legal address: Office 15, 1/2 Samarkandskaya Street, Ufa, Republic of Bashkortostan, 450105 (hereinafter – the “Operator”), under the following conditions:

Methods of Processing: The Operator has the right to carry out automated processing, as well as processing without the use of automation tools, of my personal data in documentary and electronic form, i.e., to perform actions with personal data as provided for by Federal Law No. 152-FZ dated July 27, 2006 “On Personal Data,” namely: collection, storage, clarification, accumulation, recording, systematization, retrieval, use, transfer, blocking, erasure, destruction.

Composition of Personal Data: last name, first name, patronymic; phone number; email address; name of the employing company.

Purpose of Processing Personal Data: provision of products and services to counterparties, including interaction regarding the provision of products and services.

I hereby confirm my consent to the Operator’s Personal Data Processing Policy, posted on the page at: https://uralwood.com/privacy-policy/ .

Term of Personal Data Processing: this Consent is valid for five years, or until the revocation of this Consent. The Operator has the right, after receiving a revocation, as well as after the expiration of this Consent, to continue processing my personal data to the extent that, under applicable law, no consent is required for such processing.

I hereby confirm that any of my conclusive actions (filling out a registration card on the Operator’s website, checking a box to give this Consent using the “✓” symbol, clicking the “Submit” button) constitute, in accordance with paragraph 1 of Article 9 of Federal Law No. 152-FZ dated July 27, 2006 “On Personal Data,” a sufficient form of expressing consent to the processing of my personal data and allow the parties to confirm the fact that such consent has been obtained. No additional evidence to further confirm my free will shall be required from the Operator.

This Consent to the processing of personal data may be revoked by sending a written statement by mail to the Operator’s location address, or to the email address pdn@uralwood.com, upon presentation of documents allowing for the reliable identification of the applicant. The Operator shall cease processing the personal data and destroy them within a period not exceeding 10 days from the date of receipt of said revocation.
